VillageMD Expands into Michigan to Bring Physicians a New, High-Value Delivery Model of Primary Care
Envision Medical Group joins VillageMD to launch VillageMD-Michigan, which helps providers improve care quality and cost
Chicago, Ill. (September 12, 2018) – VillageMD, a leading national primary care provider, has expanded into Michigan to help physicians statewide deliver a higher-value model of primary care and succeed in value-based arrangements. VillageMD-Michigan offers physicians the solutions and services they need to improve clinical outcomes, enhance patient and physician satisfaction, and lower the total cost of care, benefiting patients, providers and payers.
Envision Medical Group, a more than 60 provider practice with nine locations across Metro Detroit, will serve as VillageMD-Michigan’s flagship provider partner. VillageMD has grown to include more than 2,500 physicians across eight markets, and is responsible for approximately 500,000 lives and $2.8 billion in health care spending.
“VillageMD is an essential partner. They provide the right mix of clinical innovation, tools and resources that we need to work smarter so we can spend less time focused on administrative needs, and more time treating our patients on our own terms,” said Jim Schoeck, chief executive officer, Envision Medical Group. “In partnership with VillageMD, we can seamlessly transition to a value-based reimbursement model which not only benefits our practice, but benefits our patients by ensuring that they continue to receive the high-quality, cost-effective care they deserve.”
Practices will leverage VillageMD’s data analytics, physician-based care coordination, and on-the-ground support resources, all of which have demonstrated significantly greater clinical improvements at a lower total cost of care. VillageMD’s clinical care model also aids its physician partners in providing more personalized attention, education, and support to patients via integrated care teams of health coaches, diabetes educators, pharmacists, and resource coordinators to address patients’ medical, emotional and social needs.
Provider partners also gain access to VillageMD’s docOS™, a first-of-its-kind primary care-centered technology system which offers real-time access to actionable patient insights to support a more complete and preemptive approach to patient care. docOS™ extracts and analyzes patient clinical, financial and payer data from EHR’s and practice management systems, hospitals, skilled nursing facilities as well as socio-economic data sources across the care continuum.
“We are excited to bring to Michigan physicians an enhanced model of primary care, especially now when they are seeking greater resources, support and validation,” said Tim Barry, chief executive officer, VillageMD. “We look forward to helping Envision Medical Group and practices across the state make an impact on the everyday health of the communities they serve.”
VillageMD’s high-value model of care is proven to drive results across multiple markets and across all patient populations – commercial, Medicare, Medicare Advantage and Medicaid. Practices that have adopted VillageMD’s patient-centric approach have been recognized by the Centers for Medicare and Medicaid Services for high quality and high-performance metrics, such as the following: admissions and readmissions rates 20 to 45 percent lower than the community, Medicare costs 20 to 45 percent lower than the community, and commercial costs 15 to 20 percent lower than the community.
